What we do (and who we do it for)

At Catalyst Choices, we are about supporting you to live your life, YOUR WAY.

Your Choice,
Your Support,
Your Way.

We’re dedicated to making sure we are adaptable and flexible enough to give the best possible care, for whatever you or your loved one needs.

Through our services, we support all adults with additional needs, including learning disabilities, as well as older people, those with dementia and those who need a bit of support to get back on their feet after a period of ill health.

Read on to find out more about what we do and who we do it for.

Support in a Catalyst home

Whatever your service needs, we are confident that we can support your stay through our:

Residential Care – for any longer-term care needs, Catalyst Choices can provide excellent support for people as they get older. We understand that it’s important to have a home-away-from-home experience and feel, that’s why through our homes at Woodleigh and Linear Park we are continuing to custom build comfortable and familiar accommodation for the people we are care for.

Nursing Care – for anyone who needs medical support whilst in our care, we are proud to have a team of highly skilled nurses at both our Lilycross and Woodleigh sites.

Short breaks – having a short-break away can be a great way to improve quality of life and bring rich and fun experiences to all those we care for at Catalyst Choices. That’s why we have full-time short break opportunities for adults who have a physical disability, learning disability and autism.

We offer our short-break stays at our Woodleigh home, and we are currently in the process of launching a brand new short-stay service which has been co-produced along with our residents and their carers and families – exciting news on this to come!

Supported Living – for adults with a learning disability or complex needs, our supported living service puts the people we can for at the center of it. Through our Newborough and tenancy sites, our team works closely with our people in care to make sure they are living the most enriching life they can.

Support in your home

If you or a loved one want independence but with a little help here and there, we provide high-quality housing where you can feel right at home. We’ll check in on you for wellbeing, personal skills and practical help, but the rest we leave up to you! We can offer this support through:

Floating Support – if you or someone your care for live in a flat or house that is owned, rented privately, or through a housing association, we can provide floating support to check in and help out when needed.

Extra Care – Catalyst Choices’ Extra Care Housing provides care and support to older adults in specially-designed accommodation, meaning that independence can remain whilst benefitting from daily check-ins, social events, and activity and leisure opportunities.

We call this ‘your home’ because you spend your retirement in the place  you  choose, doing things  you  enjoy, with support from a team that’s there for  your  needs – all supported through our Mosslands location.

As you get back on your feet

Step-down Services – these services help you after you’ve had a bad spell. Maybe you’ve been in hospital or in another form of care: these services are about getting you back home.

We can support you with through these services with our Woodleigh Mini Stays and our Lilycross recovery centre.

To get out and about

Everyone needs much more than to simply be ‘cared for’. We need friendships, skills and fulfilment, in a place where we are understood.

Catalyst’s Day Opportunities enable people with additional learning needs and/or physical disabilities to take part in a diverse range of day-time activities, using local community facilities. This might be support with daily tasks, or treatments such as Occupational Therapy, independence building activities, as well as social and leisure opportunities.

Shared Lives

Catalyst Choices are also proud to be part of the Shared Lives scheme which also provides flexible daytime or evening support, short break opportunities (from one night to as many as you need) and, long term care – where a dedicated person moves in and lives as part of the family.

About Woodleigh

RESIDENTIAL
Based in Callands-Warrington, our purpose-built home at Woodleigh provides the highest quality home-from-home experience for older adults, adults in rehabilitation and those who require specialist dementia care. We are a friendly and vibrant community, with several events running every week by our experienced and caring team, designed to keep us moving and entertained while getting the full-time care we need.

With 39 beds on offer, Woodleigh provides CQC-rated ‘good’ quality care for people who need support with daily living activities from staff who are highly trained. A sense of belonging and fun is fundamental to our mission and each household unit offers a warm, comfortable shared living experience.

The service and the homely environment are designed for people to maintain their independence, achieve their goals and to build on meaningful and long-term relationships with other residents.
We also offer intermediate care through our ‘mini stays’ service, which provides the opportunity for recovery and rehabilitation through a planned programme of care and treatment. The aim of the service is to support discharge from hospital and living in their own homes to maximise independence, promote wellbeing and to prevent unnecessary readmission into hospital.

SHORT BREAKS
We are also proud to provide Short-Break services at our Woodleigh home. We offer full-time opportunities for adults with a physical disability, learning disability or autism, aimed at carving out a rich quality of life and varied experiences.

We will also support individuals to transition into adult services as we are with you every step of the way. We know it can be a little daunting, so this is why we can also offer taster sessions for those who might be planning to move into supported accommodation but have never spent a night away from their family.
